Build more capable WordPress forms

Custom Contact Forms Pro expands the Custom Contact Forms builder for websites that need more than a basic contact or enquiry form. It supports forms for bookings, orders, registrations, donations, waivers, agreements, and other processes where users may need to pick options, pay, sign, or receive a record of their submission.

The builder remains inside WordPress and supports drag-and-drop form creation, live previews, editable field labels, width controls, included themes, and custom CSS. Pro adds commercial workflows without requiring an e-commerce stack, while forms can still be placed with a Gutenberg block, shortcode, widget, or PHP function.

Payments and pricing

Accept payments directly within a form through Stripe or PayPal. Stripe forms can support cards, Apple Pay, Google Pay, and Link, with a live payment button and server-verified totals. PayPal payments also take place in the form with server-verified totals.

Product and pricing fields support fixed prices, multiple options, or a customer-chosen amount. You can add quantities, pictures, and enforced minimums. Coupons may use percentage or fixed discounts, while live totals update as visitors make selections. The source describes no added transaction fees beyond Stripe’s or PayPal’s standard processing fees.

Signatures, consent, and receipts

Add a digital signature field that visitors can complete with a mouse, finger, or stylus. The signature is saved as an picture with the submission. A consent or terms field can display scrollable terms, record versioned agreements, and permanently snapshot the agreement.

For paid orders, PDF receipts can be attached to notification emails. Individual submissions can also be downloaded as PDFs with one click, giving administrators a practical record of completed transactions and agreements.

Flexible form journeys

Use multi-step forms with page breaks, progress indicators, and per-step validation. Conditional logic can display or conceal fields based on answers, including fields that affect pricing. Survey and star-rating fields support Likert grids and quick ratings for feedback and evaluation forms.

Address autocomplete lets visitors choose an address as they type, filling the street, city, state, and ZIP fields. It uses your own Google API key. SMS confirmations can notify clients when they book or pay through your own Twilio account.

Templates and practical workflows

Thirteen ready-made templates cover uses such as donations, orders, registrations, waivers, and agreements. Start with a template, adjust the fields and settings, then insert the saved form where it is needed. Pro is suited to appointment requests, paid registrations, order forms, signed documents, customer surveys, and consent-driven submissions.

Installation and setup

1. Open the WordPress form builder and create a form from a template or from scratch.
2. Drag fields into the form, then configure pricing, payments, conditional logic, signatures, consent, or other Pro features as needed.
3. Save the form and place it with the Gutenberg block, shortcode, widget, or PHP function.

The AI form-building option uses the AI provider already connected under Settings > Connectors. It requires WordPress 7.0 or later, and the option remains hidden otherwise.
