Forminator Pro

Build forms around the task

Forminator Pro is a WordPress form builder with a visual drag-and-drop editor. Create contact, order, payment, email, feedback, application, login and registration forms, as well as polls, quizzes and calculations. Pro unlocks all form fields and premium add-ons from the start. Add the finished item to a page or content area, including with the Forminator block in Gutenberg.

Forms can collect details such as names, email addresses, phone numbers, files and dates. Query strings can prefill user information; email routing, automatic replies and conditions help direct submissions and responses.

Polls, quizzes and calculated results

Polls collect visitor opinions with configurable settings and can show live results in charts and graphs. Quizzes can test knowledge or use a no-wrong-answer format. Visitors can share quizzes, and an optional lead form can collect participant details.

Calculation forms use visitor input to collect information, generate leads, take orders or return configured results. Examples include:

• Product orders with size and color variations, prices and tax
• BMI, calorie, loan and due date calculators
• Service estimators and return-on-investment calculators

The result depends on the fields and calculation configured for the form.

Set up payments and subscriptions

Forminator integrates with Stripe and PayPal for fixed or variable payments, donations, down payments and merchandise orders. Enter the required publish keys before activating a payment module. Stripe’s Dynamic Payment Element can present more than 40 payment methods, including Apple Pay, Google Pay and Klarna; settings let you choose which options users see.

Forminator Pro includes Stripe subscription forms. Configure selectable plans or pricing, billing cycles, custom upsells and an optional free trial. Subscription form data can sync with the connected Stripe account.

Add Pro fields and tools

The Pro e-signature field lets visitors sign with a mouse, trackpad or finger on a touch device. Pro also includes dynamic PDF generation and geolocation with autocomplete for location-related input.

Configure and publish

Choose the form, poll, quiz, calculation or payment process, then add and configure its fields in the visual builder. Set up Stripe or PayPal with the required publish keys when collecting payments, and choose the Pro tools needed for signatures, subscriptions, PDFs or location input. Add the completed item to the relevant WordPress page or content area.
